Несмотря на широкий выбор антигипертензивных препаратов, лишь небольшое число пациентов с артериальной гипертензией (АГ) лечатся эффективно. Ингибиторы ангиотензинпревращающего фермента (АПФ) рассматриваются как препараты первого выбора для стартовой терапии неосложненной АГ Среди множества представителей класса ингибиторов АПФ особого внимания заслуживает лизиноприл, который является одним из наиболее известных и хорошо изученных препаратов этой группы с широким спектром действия и доказанными органопротективными свойствами.

Aim: development of criteria and a method of prognosing of emergence of adverse side effects to the first mode of chemotherapy at patients with for the first time revealed tuberculosis. Materials and Methods: The analysis of the collateral reactions (CR) on tubercular preparations has been carried out at 214 patients receiving treatment at the first mode of chemotherapy. During the treatment an indicator of quality of life by a technique the DIGNITY (health, activity, mood), a condition of intersystem interaction of respiratory and cardiovascular systems of an organism (Hildebrant's coefficient), a vegetative index of Kerdo were estimated at patients. Results. Ufavorable effects on PTP develop at every third patient (33,6%). The CR different types depends on the age and availability of the accompanying pathology. Allergic reactions develop at patients with existence of endocrine pathology (27,8%) authentically more often, toxic — at patients with defeats of nervous system (56,3%). Extent of change of an index of Kerdo of 59,4±2,4%, Hildebrant's coefficient 48,9± 1,6% in the first 4 weeks of treatment of the patient corresponds to emergence of CR in the first 3 months of chemotherapy. Conclusion. Dynamics of coefficient of Hildebrant, Kerdo's index and an indicator of treatment, SAN at the initial stages, are considerably associated with the risk of development of side effects of chemotherapy and were a basis of the developed computer system expert for prognosing the development of these complications. The predictive value of system is 76,3% of sensitivity at 84,7% of specificity.

Aim: studying of clinical and functional features of juvenile hemangioma to identify indications to their removal. Analysis of the nature of side effects at the different methods of treatment and determination of efficiency of the ra-diowave moving away of juvenile hemangioma. Materials and methods. The study involved 216 children with juvenile hemangioma and 14 adult patients with persistent hemangioma. Radiowave treatment was conducted to 32 children with superficial juvenile hemangioma. Results. Superficial juvenile hemangioma are diagnosed in 76.1%, thus the highest prevalence of superficial juvenile hemangioma of small diameter and thickness. At the inspection of children with complications of different methods of treatment of hemangioma found the prevalence of formation of pathological scars, during removing of hemangioma by carbon-dioxide laser and at a scelerotherapy — 58,3%. Swelling and soreness at the radio wave moving away at most patients (84,3%) passed through 3 days, recovery after a 1 session came at 65,6% of patients, after 2 sessions — at 28,2%, after 3 sessions — at 6,2%. At watching the patients of this group during one to three years their proof recovery is set. Conclusion. High postoperative cosmetic effect under strict indications for surgical removal, the absence of relapses and the low percentage of continued growth (6.2% after the second session deletion) allow to recommend high-frequency radio wave removing for the treatment of superficial juvenile hemangioma of the skin.

It is familial benign chronic vesicular fever (Hailey-Hailey disease) in a 58-years old female patient which is inherited as an autosomal dominant mode. The data about etiology, pathogenesis, clinical picture and differential diagnosis were summarized. The main ways of treatment of the disease are described.

Objective: to assess clinical efficiency and safety of using long wavelength ultraviolet therapy in children children and adolescents suffering from atopic dermatitis during exacerbation of the dermatosis. Materials and methods. The data about the usage of the far long-wave UV therapy in the treatment of 38 children suffering from atopic dermatitis. The 2nd group of patients (42 patients) received standard therapy with the use of the usual doses and methods of taking medication and did not differ from the first group by age of patients, duration and form of the disease. Results. All observed on the background of therapy was noted positive dynamics of the pathological process in the skin, however, patients of the 1st group were observed earlier (average 5-7 days) than in the 2nd group, resolution of the clinical manifestations of AD, which was confirmed by a significant decrease in SCORAD values (1,9±0,7; p<0.05). Conclusion. The high efficiency of the far long-wave UV therapy, as evidenced by a reduction of the absolute values of SCORAD index.

The aim of the research is to improve the efficiency of diagnosis and treatment of NSAID-induced enteropathy at elderly patients with chronic pain (osteoarthritis, dorsopathy). Material and Methods. 300 elderly with osteoarthritis or dorsopathies regularly (8 weeks or more) receiving diclofenac at 75 mg per day were inspected. Clinical features, morphology, components of the diffuse endocrine system of the small intestine were studied. Results. It is defined, that at elderly patients with regular use of NSAIDs in 25% of cases postbulbar erosive duodenitis, in 75% of cases inflammatory and atrophic changes of the distal duodenum were diagnosed. Diagnosis of NSAID-induced enteropathy at elderly should include faecal calprotectin and morphology analysis of postbulbar area of the duodenum. Use meloxicam and sulfasalazine at older patients helps to eliminate or reduce intestinal dyspepsia, malabsorption, and intestinal inflammation on the results of faecal calprotectin and of histology of the small intestine. Conclusion. Elderly patients are at high-risk group for NSAID-induced enteropathy due to functional disturbances in the neuroendocrine system of the small intestine.

Research objective: to estimate results of treatment of patients with the localized form of a cancer of a prostate at a combination of a transurethral resection (TURP) and HIFU of an ablation. Objects and research methods: From February, 2009 to February, 2014 of 100 patients with the localized form of a cancer of a prostate were selected for research: 26 patients were included into HIFU and 74 group in group of the combined treatment (TURP+HIFU). Selection criteria for HIFU ablation were the localized cancer of a prostate concerning which earlier it wasn't carried out treatments, and level of a PSA at the time of statement of the diagnosis 15 ng/ml. All patients corresponding to these by criteria, were considered as candidates for treatment and inclusion in the analysis. The nadir and stability of PSA, the histologic conclusion, IPSS, quality of life and complication were estimated at time of postoperative supervision. Results: Statistically significant influence of a combination TURP+HIFU for the term of transurethral drainage of a bladder (a median of 40 days against 7 days), incontience frequency (15.4% against 6.9%), infections of urinary ways (47.9% against 11.4%) and IPSS change during the postoperative period (on the average 8.91 against 3.37) is noted. During the short period of supervision it wasn't observed considerable changes in relation to efficiency: in HIFU group the frequency of repeated sessions made 25%, in TUR/HIFU group 4%. Conclusion: HIFU therapy is modern, minimum invasive method of a cancer therapy of a prostate. The combination of a transurethral resection and HIFU ablation significantly reduces the frequency of the complications connected with treatment. Maintaining the patient after combined TURP and HIFU ablation is comparable with maintaining the patient after usual TURP.

Background — The presence of chronic hard-to-heal wounds significantly affects patients’ quality of life causing pain, discomfort, decrease of mobility and consequently leads to social isolation and depression. This type of wound is more prevalent among older population. It has been estimated that up to 1-2% of the world population have this condition. The treatment of hard-to-heal wounds is expensive and long-term process, and, stresses healthcare systems of every country. Unfortunately, limited information is available about the situation with chronic wounds in Kazakhstan as the statistical data on such patients is not monitored and not registered in this country. Methods — The search was conducted by using available electronic sources, including Google Scholar, PubMed, Web of Sciences, Medline, Wiley Online Library, and Cochrane Library databases as well as hard copy versions of peer-reviewed publications in Russian, Kazakh, and English languages. Results — We have identified and analyzed current situation with wound care in the Republic of Kazakhstan. Research has demonstrated a variety of types of treatments utilized in the country, particularly the treatment of the diabetic foot.It also indicates the lack of statistics on wound care. Conclusions — The review highlights the mechanisms of wound healing process, methods for wound care, and encompasses the information available on wound healing in Republic of Kazakhstan. The article emphasizes the necessity of establishing the systemic monitoring of wound care and formation of electronic database. Apart from that, the importance of developing and manufacturing of domestic novel wound treatments have been also stressed out.
