The purpose of this work is to define the clinical and social predictors of headaches in children and adolescents. Material and Methods. We performed clinical examination of children and adolescents, which included studying their symptoms, anamnesis, somatic and neurological objective signs, survey using our original questionnaire and survey in the framework of an international project to study headaches «World Children and Adolescent Headache Project». The type of headache was diagnosed using international diagnostic criteria of the International Classification of Headache Disorders, 3rd edition (beta release). As a result of our study, we obtained the data about the most significant lifestyle factors (among those studied) influencing the number of days with headache per month, calculated linear coefficients and a predicting mathematical model for children. The sensitivity of the model for the prognosis of chronic tension type headache is 63%, specificity — 81 %. Conclusion. In this paper we describe criteria for the prognosis of primary headaches in patients aged from 8 to 18 years old.

The aim of the research is to identify common patterns of compensatory changes in internal architectonics of the proximal femur with valgus and varus deformity. Material and Methods. The parameters of the proximal femur were determined on the basis of 78 roentgenograms of children with hip dysplasia (1-4 years, 4-7 years, 7-16 years) before and after surgical treatment (after 6 months). Results. The degree of change in internal architectonics of the bone substance has been determined after changing of the femoral neck-shaft angle achieved with the help of correcting osteotomy in different age groups. Conclusion. Maximum recovery of internal architectonics of the bone substance is observed in the groups aged 4-7 years.

Objective — Minimally invasive repair of pectus excavatum (PE) has been established as the preferred technique for the repair of PE. Modified techniques of repair based on D. Nuss method for PE are promoted to improve the safety of the procedures. Purpose of this work was to analyzing the operative results with accounting of the toracometric parameters of pectus excavatum in children and adolescents. Methods — Toracometric parameters were the followings, as: sternum rotation, cardiac and pulmonary vein rotation angles, Haller and asymmetry indices in children and adolescents with PE. 48 (64%) patients are operated by D. Nuss procedure and 27 (36%) with modifications, due to severity of diagnostic parameters, which were exhibited the significance difference between pre- and postoperative indices, mainly, in patients who has a severe degree changes preoperatively. Results — The sternum rotation degree (in 75 patients) was decreased till 0.4±0.02° (P=0.01), whereas was 20.7±0.46° preoperatively (P=0.001). Preoperatively, the mean of cardiac rotation angle significantly reduced, preoperative was 56.0±2.9° (P=0.001) (in 75 patients) and after operative intervention made up 47.0±2.7° (P=0.001). Haller index also is decreased till 2.3±0.2, whereas was 3.9±0.2 preoperatively (P=0.002). The angle of the pulmonary vein rotation was preoperatively 51.0±3.4° (P=0.001) and after operation was increased till 53.0±1.8° (P=0.01). Conclusions — It has necessity apply the computer tomography investigation in patients with PE deformity. Also, it cannot miss the stage of the sterno-costal complex strength, its pliability and the gravity of the sternum rotation, which may lead to raise the share of good and excellent results.

After so many years devoted to practicing medicine as children physiotherapists, the therapists finally found the importance of balance training exercises in children who suffer from cerebral palsy. It is only through controlling balance that we can achieve improvement in body movement and position that will culminate into performance independence in a child. Formerly, working over controlling balance in cerebral palsy children has been very difficult, because contracture and spasticity did not let us to have an effective balance training exercise. In this respect, we have summarized the results of previous authors that specify the level of effectiveness of exercise therapy. The results of different studies showed that level of effectiveness of exercise therapy on alleviating the symptoms of spastic cerebral palsy is average in accordance with Cohen's effect size Interpretation table.

Hemangiomas are the most common tumors of skin and soft tissues in children. In recent years, the number of patients suffering hemangiomas becomes increasing. Aim: to evaluate the efficacy of non-selective (3-blockers in treatment of hemangiomas in infants with short duration of the disease. Materials and methods. Propranolol, a non-selective (3-blocker, was chosen as a medication. Observed group was composed of 5 infants aged 1 year. Starting dosage of propranolol was 2-4 mg, with hemodynamic parameters being controlled before and after the drug administration. Target dosage was 2.0-2.5 mg/kg. Results. All children had a distinct clinical improvement after 1 week of propranolol treatment. In 4 infants Doppler mode ultrasound scan of hemangiomas demonstrated the hollowing of a feeding vessel. Resistance index was reduced in 4 infants. Shrinking of hemangiomas was stated in 2 infants during ultrasound scanning. All infants continue to be observed by surgeons and pediatricians. The drug is well-tolerated. Adverse effects during the treatment with propranolol have not be noticed, both during the in-patient and out-patient phase. Conclusion. Using propranolol as the first-line drug opens new possibilities in the conservative therapy of hemangiomas when surgical intervention is not possible.

Aim: to evaluate the efficacy of pharmacotherapy of bronchial asthma in children with concomitant correction of magnesium deficiency. Material and Methods. A 12-week, prospective, randomized, open, comparative, parallel-group, which was included in the 50 children with uncontrolled and partially controlled atopic asthma and laboratory-confirmed magnesium deficiency. For the groups 1 and 2 it was assigned basic therapy of asthma in accordance with the recommendations of GINA (2011). In the group 1 it had been additionally performed concomitant correction of magnesium deficiency of Magnesium B6 Forte. There had been evaluated the effectiveness of a drug therapy for 12 weeks on the achieved level of asthma control, the frequency of exacerbations, the number of asymptomatic days. Dynamics of the severity of allergic inflammation was assessed by monthly monitoring of levels of nitric oxide in exhaled air (FeNO). Results. After 12 weeks of treatment, the magnesium content in erythrocytes in group 1 (1.79 (1,68-1,89) mmol / L) increased by 15.7% and was statistically significant (p <0,001) differed from group 2 (1.63 (1.57-1.68) mmol /L). By the end of the study the number of controlled asthma episodes in group 1 was significantly (p = 0.038) larger (20/25) than in group 2 (12/25), and the content of FeNO (19 (13-24) ppb) — significantly less (p = 0.013) than in control group (25 (21-30) ppb). Conclusion. The efficacy of pharmacotherapy of bronchial asthma in children is increased by the background correction of magnesium deficiency.

effectiveness of basic therapy for atopic asthma in children. Material and Methods. A 24-week, prospective, comparative, randomized, open-label study in parallel groups was performed. There were included 70 children with uncontrolled atopic asthma with high FeNO (FeNO>50ppb). There were evaluated the comparative efficacy of selection of basic therapy by using monthly monitoring of FeNO and criteria of asthma control in accordance with the recommendations of GINA (2011). Results. The cost / effective ratio (CER) for achievement of one permanent 12 week-control after 24 weeks of therapy in group of monitoring FeNO was 11433,26 rubles, which was 40% lower, than the CER in the comparison group (18382,99 rubles). Conclusion. The using of FeNO monitoring for a personalized correction of basic therapy in children with uncontrolled atopic asthma can reduce the cost of treatment and is also economically viable.

This article represents results of pharmacoepidemiological research of H.pylori eradication treatment of adults and children in Volgograd. The aim was to determine H. pylori pharmacotherapy stereotypes and compare with international experts' recommendations. Material and Methods. There were analyzed CRFs of 94 adults and 132 children with H.pylori associated diseases in Volgograd region. Results. There were identified mistakes of treatment regimes and eradication control principles. Conclusion. The treatment of H.pylori infected adults and children in Volgograd does not match to international experts' recommendations.
