Based on the published data the role of inflammation in development of pneumonia has been discussed. The role of inflammatory mediators and laboratory markers for diagnostics of community acquired pneumonia is evaluated, as well as prognostic role of different biomarkers of inflammation, available for clinical laboratories, but not widely used in daily clinical practice. Influence of Omega-3 polyunsaturated fatty acids on inflammatory processes and possibilities to use them in the treatment of community acquired pneumonia is also discussed in the study.

Community-acquired pneumonia (CAP) is usually characterized by absence of clinical signs. Rapid diagnostic tests with high sensitivity for lower respiratory tract infections (LRTI) could lead to improved patient care. 97 servicemen with LRTI were analyzed to compare radiological, clinical and transresonant functional topography (TRFT) findings in the differential diagnosis of CAP The research data revealed that TRFT findings improve the assessment of pneumonia based on carefully taken case history and physical examination. TRFT data contributed to development of treatment course algorithm in case of acute respiratory tract infection

Qualitative assessment of diagnostics and treatment of patients with community-acquired pneumonia (CAP) was carried out in ten therapeutic departments of urban and district hospitals of Saratov region, using quality indicators (Qls). Each case of CAP was assessed in expert health care quality (HCQ) card according to the diagnostic and treatment quality federal standards and the basic Ql. The application of Ql in CAP patients allowed revealing the following: low CAP agent isolation rate; late hospitalization of CAP patients; low frequency of sputum bacteriological and bacterioscopic investigations prior to antibiotic therapy; insufficient frequency of step-by-step introduction of antibiotics. Recommendations on CAP patients dispenserization were not properly prescribed; recommendations on vaccination were absent. The basic measures of HCQ improvement are as follows: organizing for each CAP patient therapeutic and diagnostic quality control according to Ql; quick administrative decisions; using of HCQ expertise during the first 2-3 days of treatment to correct diagnosis and treatment; optimizing hospital diagnostic resources (laboratory and instrumental) and rational pharmacotherapy

The article presents the research findings concerning the importance of changes of the contents in blood serum of interferon gamma (INF-γ), interferon alfa (INF-α), the tumor necrosis factor alfa (TNF-α), interleukin 4 (IL-4), interleukin8 (IL-8) and clinical parameters at patients with community-acquired pneumonia before the end of treatment (basic therapy and with immunomodulator tiloron). Tiloron positive influence on clinical manifestations and immunological parameters (profile of cytokines– IL-4, IL-8, INF-α, INF-γ and INF-α) has been detected in patients with slightly complicated community-acquired pneumonia in 95 % of cases.

Clinico-epidemiological analysis has been performed to reveal exogenic and endogenic riskfactors of community acquired pneumonia military servicemen at conscription. Riskfactors of CAP at conscription include smoking, overcooling, chronic infection foci, preceding ARVI, and the first-half-year of service. The residual effects after CAP in military servicemen predominantly are the obstructive disorders of outer respiratory function as well as radiological symptoms as deformity of pulmonary picture and pleural overlay. The educational scheme has been developed for CAP patients. No recurrent infections of the upper and lower respiratory passageways have occurred during the following years of military service in patients under the follow-up care.

Practical advantage of C-reactive protein determination over procalcitonin one has been demonstrated. The trans-resonant functional topography in comparison with low exposure digital fluorography technique has been carried out.
