The aim of the study was to provide the data on the course of myocardial infarction (Ml) associated with diabetes mellitus (DM) in hospital. Material and methods. 102 patients with Ml were included in two cohorts, first with DM, second — without DM. Groups were compatible by gender, age and standard therapy. Results. Angiocoronarography data have revealed that patients of group 1 were characterized by multiple vessels lesion due to "rescue" angioplasty with stenting performed 5 times. Patients with DM demonstrated in-hospital higher rate of both acute (by Killip) and chronic (by NYHA) heart failure of advanced degrees as well as recurrent Ml, early postinfarction angina pectoris, heart rhythm disorders and hematomas due to injections. Conclusion. The previously mentioned results lead to higher mortality in this group. Significant elevation of CRP, interleukin-6 and epidermal growth factor levels proved more activated systemic inflammatory response in patients with Ml and DM playing key role in pathogenesis of complications of the disease.

Aim: examination of professional qualification of NPP personnel suffering from post-infarction cardiosclerosis, to work with sources of ionizing radiation with the formation of proposals of amendments to the order of the health Ministry from 12.04.2011 No. 302-N. Material and Methods. Medical database by making indicators with individual registration forms made from 269 persons with postinfarction cardiosclerosis staff of main and auxiliary production of Russian NPPs (272 men and 7 women), average age of the examined patients was 56.2 ± 7.0 years; the total number the analyzed characteristics, reflected in the generated database was 49 lines. Results. The results of the risk stratification of sudden cardiac death with the formation of risk groups and proposed draft amendments to the Order of the health Ministry of the Russian Federation of 12 April 2011 No. 302-n order differential approach to the assessment of competency in hazardous conditions patients with postinfarction cardiosclerosis. Conclusion. The aim of the study is to evaluate the frequency of occurrence of leukocytosis and its prognostic value for the course and outcome of myocardial infarction with ST-segment elevation. Material and Methods. The study included 245 patients aged 61,2 years, who were on treatment at the Department of Emergency Cardiology with diagnosis of acute myocardial infarction with ST-segment elevation. Results. Leukocytosis (white blood cell levels of more than 10*109/L) was observed in 34,7% of patients. The presence of leukocytosis was associated with increased incidence of congestive heart failure class IV at Killip, ventricular fibrillation and lethal outcomes during hospitalization. There were no significant differences in the incidence of adverse events in the long-term period between the patients with leukocytosis and with normal level of white blood cells. Conclusion. Increased levels of white blood cells (more than 10*109/L) in patients with myocardial infarction with ST-segment elevation is associated with a significantly increase relative risk of cardiogenic shock (Odds ratio 5,2, 95% Cl, 1,7-15,8, p=0.001), ventricular fibrillation (Odds ratio 8,5, 95% Cl, 1,9-38,3, p=0.001) and death during hospitalization (Odds ratio 2,47, 95% Cl, 1,87-38,4, p=0,03). The level of white blood cells on admission to hospital may be used as one of the additional factors predicting the risk of patients with myocardial infarction with ST-segment elevation during hospitalization.

Objective: to establish the relationship of chronic generalized periodontitis, multiple cavities and dentofacial anomalies with various forms of coronary heart disease and its complications. Material and methods. The study included 294 patients with coronary heart disease and its complications. The patients were divided into two groups. Group I included 89 patients with acute Q-myocardial infarction, which was placed in the presence of at least 2 of the following criteria identified on the basis of a comprehensive clinical and instrumental examination: clinical, laboratory confirmation (CK-MB), electrocardiographic signs of damage or myocardial necrosis. The following statistical methods were used: multi-variate and univariate analysis of variance, non-parametric tests, crosstabulation, chi-square test, Fisher»s exact test. As a measure of variability of the normal distribution standard deviation was used. Results. It is noted that in patients with myocardial infarction more likely than in patients without coronary heart attack in history severe generalized periodontitis, dentofacial anomalies and multiple dental caries have appeared. In patients with acute myocardial infarction, severe periodontal disease has been associated with increased fibrinogen levels in the blood and an increase in the dispersion of the interval QT, which are known to be indicators of poor prognosis in acute coronary disease.

The aim: of this study was to evaluate prognostic value of autonomic regulation indices for the 5-year risk of fatal cardiovascular events in patients after acute myocardial infarction (MI). Methods: We studied 125 patients (53 female) after acute MI aged 30-83 years. The period of observation was 5 years with checkpoints at the first week after acute MI and then annually. Synchronous registration of electrocardiogram (ECG), photoplethysmogram and respiration were made in rest in all patients. The presence of synchronization of 0.1 Hz rhythms in cardiovascular system (CVS) (heart and microcirculation) were determined from the difference of phases. The degree S of 0.1 Hz rhythms synchronization were estimated. Results: We compared prognostic value of different degrees S of synchronization between 0.1 Hz rhythms in CVS for evaluation of the 5-year fatal risk in post-MI patients. S < 20% at the first week after acute MI were identified as the most important factors for evaluation of the risk of 5-year mortality in post-MI patients (χ2 = 10.5, P = 0.005). Sensitivity and specificity of S < 20% at the first week after acute MI were 76% and 43%, respectively. Conclusion: The degree S of 0.1 Hz rhythms synchronization below 20% in post-MI patients is a sensitive marker of high risk of mortality during the subsequent five years.

Cryoinfarction in rats was carried out by placing the cooled probe directly on the midway of left anterior descending coronary artery. This caused damage to the blood vessel, hindering blood flow to the distal part of the left ventricle. Gross pathology showed around 26 % infarction at 24 h. Histopathology revealed death of cardiomyocytes with blood vessel congestion at the end of 24 h, inflammatory infiltrate at 48 h, fibrotic scar by 96 h and collagen deposition by 192 h. Acute myocardial infarction biomarkers such as Cardiac Troponin T, Creatine Kinase MB and NT pro BNP were shown to be elevated by 4 h. ECG showed an ST segment elevation by 96 h. This cryoinfarction model was suitable to study changes taking place during acute myocardial infarction in humans.

The research goal is to evaluate the impact of autonomic heart control indices on five-year risk of lethal outcome and development of cardiovascular disorders in patients with acute myocardial infarction (AMI) and to study the dynam¬ics of indices in short- and long-term period. 125 patients with AMI (42% female), ages 30 to 83 years, were enrolled in prospective observational study. Observation period lasted 6 years. Control checking was carried out in 1 week, 3 weeks, 6 months, 1 year and then annually. The dynamics of heart rate variability (HRV) and the synchronization of 0.1-Hz rhythms of heart and microcirculation (S) were studied during the first year. Outcomes proved to be death, MI, stroke. Development of acute heart failure Killip 2-4 and indices S < 20% during the first week of AMI (х2 = 10,5, p = 0,005 for the Cox model) played a significant role in evaluation of five-year risk of death after AMI. Sensitivity (Se) and specificity (Sp) of index S < 20% during the first week of AMI were 76% and 43% correspondingly. The Cox model showed that indices of HRV and 0.1-Hz rhythms synchronization were not of great value in evaluation of five-year risk of death after AMI. The article concludes that indices S < 20% in patients with AMI possess better prognostic value than common clinical parameters
