A new methodological approach has been proposed according to the insufficient results of hypertension (HD) and atherosclerosis (AT) treatment and preventive measures. This method is characterized by the determination and taking into account the phases of systematic chronic inflammatory process in vessels and concentrates on the conduction of an active anti-inflammatory therapy (AIT). Special attention has been paid to the diagnostics and treatment of different phases of interseizure period (ISP), mainly of preactive phase. These measures should both prevent activation and progressing of the process and become a kind of prophylaxis

The research goal is to point out potentiality of clinical audit of care quality carried out by IAS "Register AH" in hypertensive patients followed in Saratov polyclinic. Data of medical cards of 205 hypertensive patients (average age 52,8±9,1 years) followed in Saratov polyclinic have been analyzed with the help of IAS "Register AH" indicators set. Implementation of clinical recommendations on diagnosis, treatment and following-up of hypertensive patients from 2007 till 2008 has been assessed. There was no correspondence between statements of recommendations of diagnostic measures for clinical status, treatment and following-up of hypertensive patients. Clinical aim of treatment to support the principal blood pressure has not been achieved in 85% of patients. The main cause has consisted in the patient's clinical status without taking into account the treatment and dynamic follow-up. Clinical audit has allowed the complex evaluation of medical care process in hypertensive patients followed in the studied polyclinic and allowed to reveal a cause of inappropriate results of care. The use of IAS "Register AH" systematizes medical data contained in ambulatory cards, automatizes the process of obtaining indicators results. It increases the reliability assessments

A new follow-up technology of dynamic out-patient control based on system of mobile monitoring of patients with arterial hypertension (SMMAH) has been created in Saratov Scientific Research Institute of Cardiology. SMMAH is based on exchange of information between patient and doctor with the help of Internet and standard short mobile messages. The aim of the present work is to study the effectiveness of SMMAH for follow-up control of hypertensive patients for the period of 12 months. 79 patients with hypertension aged 49±11 were included in the investigation. Control period lasted 12 months. Control visits were made in the 1st, 6th and 12th months. The number of patients became out of control, its causes, percentage of patients who achieved and maintained target blood pressure (BP) were analyzed after each visit. Data were presented as M (95% confidence interval). As a result in 1 month period of investigation 88,6% of patients regularly used mobile messages, 11% of patients were out of control. In 6 months period 68,4% of participants were under control, 31,6% of patients dropped out. In 12 months period — 67% and 33% of patients accordingly. Target BP was maintained in 87% (77%-96%) of patients in 1 month period, in 78% (66%-90%) of patients in 6 months period and in 68% of patients (53%-84%) in 12 months period. High clinical effectiveness of SMMAH has been proved during the study: 67% of patients have followed prescribed therapy after the investigation period and 68% of them have maintained target BP level.

Analysis of transmitral flow criteria has been used to underestimate diastolic dysfunction degree. The study goal was to compare the evaluation of different echocardiographic methods to determine diastolic dysfunction types. There were 120 patients with chronic cardiac insufficiency under the study; control group included 30 healthy individuals. Pulmonary venous flow, transmitral flow at rest in carrying out Valsalva test and tests with isometric exercise have been examined for verification of diastolic dysfunction type by Impulse Doppler Echocardiographic Method. The findings have been compared with the results of Tissue Doppler Echocardiography. Among patients with diastolic dysfunction type I according to the transmitral flow examination there were 22 (34%) with diastolic dysfunction type II using Tissue Doppler Echocardiographic method. Besides, 6 persons with diastolic dysfunction type III demonstrated no diastolic dysfunction signs according to the transmitral flow data. The Valsalva test enabled differentiation of diastolic dysfunction type II in only 39% of all cases while test standardization increased the method’s value up to 55%. The isometric exercise test for diastolic reserve indication may be compared by its effectiveness with Tissue Doppler Echocardiographic method used for detection the diastolic dysfunction restrictive types.

Expiration function, cardiohemodynamics and blood viscosity indices have been investigated in 178 patients in order to evaluate the efficacy of plasmapheresis application in complex therapy of acute bronchial asthma combined with arterial hypertension. Complex therapy proved to be advantageous.

The aim of present work is to study the frequency of hypertension and the peculiarity of 24-hour blood pressure monitoring in patients with rheumatoid arthritis. We studied 584 patients with rheumatoid arthritis. Daily blood pressure monitoring was carried out in 46 patients with arthritis and 18 patients with essential hypertension. Frequency of hypertension was higher in patients with arthritis then in general population. Elevation of systolic blood pressure, heart rate and arterial blood pressure adequate decrease at night were developed to associate with advance of the disease.

Current article shows results of psychophysiological checkup of 266 clinically healthy people and of 64 patients with arterial hypertension (average age 23,54±7,05years old), from different social status categories. Was determined that all clinically healty people were choosing adaptive forms of coping behaviour and had satisfactory adaptation reserves of cardiovascular system (except certificated medical personnel, who had adaptation mechanisms stresses). For patients with arterial hypertension selection of non adaptive form of coping behaviour and fall of adaptation reserves of cardiovascular system was typical.

We have compared the daily blood pressure monitoring data with arterial wall rigidity parameters in patients with osteoarthritis with and without arterial hypertension. In 32% of patients with osteoarthritis without arterial hypertension we have determined “non-dipper” type and in 16% of patients - “night-peaker”. In case of arterial hypertension 42,8% of patients with osteoarthritis have “night-peaker” type and 22,8% of patients have “non-dipper” type. We have found the correlation between augmentation indices and daily blood pressure parameters.

Peculiarities of arterial hypertension in patients with bronchial asthma give rise to the scientific and clinical interest for investigators. The aim of the present study was to elucidate and compare special characteristics of systemic hypertension in patients with bronchial asthma and arterial hypertension by means of daily blood pressure. 93 patients with bronchial asthma were under study (34 men and 59 women of 40,2 ± 8,93 years old): 21 patients without hypertension, 14 patients with primary hypertension and 58 patients with pulmonary hypertension. 77 patients with essential arterial hypertension (30 men and 47 women of 41,7±4,8years old) constituted the control group. Daily blood pressure monitoring was carried out in all patients. Insufficient night decrease of blood pressure (<10%) in patients with asthma and pulmonary hypertension was twice as much than in patients from the control group (53,5% and 26% accordingly). Elevation of blood pressure, heart rate, blood pressure variability, pressure load indices were developed to associate with frequency of dispnoea attacks, especially in patients with bronchial asthma and pulmonary hypertension.
