Soy bean trypsin inhibitor (SBTI) belongs to the family of serpins – serine protease inhibitors widely distributed in the nature. Serpins participate in the regulation of proteolytic reactions underling very important physiological and pathological processes such as digestion, blood clotting, immunity, apoptosis, inflammation, carcinogenesis and so on. Despite the apparent importance of the correction of imbalances of the proteolysis in pathology, in fact only the pancreatic trypsin inhibitor (aprotinin) is used more or less widely as a protease inhibitor drug (Trasisol, Contrical, Gordox etc) in the treatment of some diseases. Being the animal protein aprotinin possesses substantial disadvantages and attempts to develop new drug on the base of plant or recombinant proteins or peptidomimetics are undertaken. SBTI is one of the candidates for such a role.

The central part of brain infarction (Bl) pathogenesis is hemostatic blood activation. Aim: to study platelet aggregative ability in dynamics of acute Bl at the marked stenosis of cerebral arteries (>50%). Materials and methods. Investigation included 39 patients at acute Bl with average age 73,9±8,3 years. Control group consisted of 18 healthy persons with average age 60,2±6,8 years. Evaluation of platelet aggregation was performed with application of light-transmission aggregometer «SOLAR» (RB) and aggregation inductors — solutions of adenosine diphosphate with concentration of 0,5 и 2,0 umol/l and collagen of 2,0mg/ml concentration. Results. Significant hyperaggregation platelet reaction on the 1 st and 10th days of hospitalization without positive changes under treatment has been revealed. Conclusion. Thus, obtained results prove the necessity of monitoring of individual patients sensitivity to antiplatelet therapy in secondary prevention of ischemic stroke with optical aggregometry and inductors complex.

Purpose. The purpose is to study the reaction of homeostasis system in stable and in distraction llizarov transosseous ostesysnthesis of the limbs bones at experimental animals (dogs) in highland conditions. Material and Methods. 63 adult mongrels aged from 1 to 3 years with 10-20 kg body weight and 15 cm minimal length of tibia were included in the experiment. Open osteosotmy of tibial mid-shaft followed by Kirschner wires insertion through tibial bone and llizarov frame assembly comprising 3 or 4 rings was performed under IV anaesthesia at 3200 m altitude above sea level. In experimental group including 33 mongrels the distraction was performed on the 5th day after osteotomy by 1 mm rate divided into 4 times within 10 days but in control group comprising 30 dogs the distraction was not provided. Every 10 days after distraction blood samples were taken in both groups to examine coagulation; the samples were taken from subcutaneous lateral tibial vein, ex tempora mixing with 3,8% solution of sodium citrate at 9:1 ratio. Then the plasma was separated and examined biochemically according to the accepted standards. Results. We obtained the data witnessing activation of coagulation component of homeostatic system in stable osteosynthesis of the limb bones using llizarov fixator with simultaneous inhibition of fibrinolytic component. In distraction osteosynthesis hyper-coagulation trend in homeostasis system with simultaneous suppression of fibrinolytic blood activity is vividly defined within the entire experiment period. Conclusion. Transosseous distraction osteosynthesis in highland condition causes considerable shifts in the system of homeostasis with definite domination of coagulation component which promotes formation of pre-thrombotic condition. Limb segment lengthening on the background of pre-thrombotic condition increases the risk of thromboembolic complications in highland conditions and requires appropriate measures of prophylaxis

The research goal was to study the influence of extremely high temperatures on the hemostasis indices in patients with uncomplicated pregnancies with the concomitant limitation of liquid intake. The research results of hemostasis of pregnant women being hospitalized during the period of spring and summer in 2010 have been analyzed. The differences in the level of hypercoagulation changes of hemostasis in summer and spring have been found to be insignificant. In the platelet link hemostasis in summer of 2010, as compared to the spring, the increase of platelet aggregation and amount has been observed, with the exception of the third trimester, which is marked by the decrease of platelet amount. Hot seasons and liquid intake limitations make additional demands on hemostasis adaptive capacity, increasing the risk of thrombohemorrhagic complications

The article presents modern data concerning mechanisms of uterine bleedings in undeveloped pregnancy, hystero-myoma, and cervical carcinoma. Hemostasis system malfunctions appeared in case of dead foetus in uterus have been considered in the work. They are manifested by consumption coagulopathy and haemorrhagic complications. Uterine bleedings in case of hysteromyoma are caused by dilation of arterial vessels and deprivation of their contractile function and malfunction myometrium contraction. Complications that occur in oncological uterine bleedings arrest have been described. They are caused by individual uterine blood supply and developed network of collateral anastomoses between branches of internal iliac arteries

Research objective. Study of condition of hemostasis system at patients with hysteromyoma complicated by bleeding; comparative estimation of hemostasis indicators at patients with uterine arteries embolization (the basic group n=48) and abdominal hysterectomy (comparison group n=65). Materials and methods. Complex study of the basic hemogram parameters and hemostasis at 162 patients with hysteromyoma and 30 patients without marked genital pathology (the II control group) was carried out. 48 patients formed the main group which was subjected to uterine arteries embolization for bleeding break; 65 patients (the comparative group) were surgically operated upon abdominal hysterectomy; 49 patients with hysteromyoma had no bleedings and formed the control group. Research of hemostasis system was carried out by means of test-tube method, and also by plotting set for express-diagnostics, analysis of thrombocyte unit of hemostasis system was realized with 2-channel analyzer of thrombocyte aggregation «Biola» — LA 230. Results. Presented analysis of hemostasiogram parameters allows to state development of chronic DIC-syndrome in women with hysteromyoma. Reliable shortening of coagulation index, decrease of thrombocyte aggregation function in comparison to the control group data were revealed. Conclusion. Development of hyper coagulation with increase of paracoagulative products with simultaneous activation of fibrinolysis processes in all the patients with hysteromyoma was revealed. Minimal effect of embolization of uterine arteries to hemostasis system in patients with hysteromyoma was marked. And they were intensified after surgical treatment, which is the evidence of definite advantages of this method unlike of hysterectomy

Prophylactic management of small doses of fresh-frozen plasma to women in the case when the hemorrhage is less than 20 % of circulating blood volume and the adaptable physiologic changes of hemostasis have been already formed, does not change its coagulative potential. The optimization of an individual program of transfusion therapy should be based on the results of screening and monitoring of hemostasis state.

The aim of this study is to evaluate season pattern, severity and outcome of acute myocardial infarction in dependence of yearly patterns of function of hemostasis in middle Volga Region. Season pattern of prevalence myocardial infarction with winter peak was discovered, be accompanied of increase platelet aggregation, deterioration of blood viscosity, fibrinolysis depression. Most serious clinical course of summer myocardial infarction was observed with prosperity of hemostasis.
