Обзор посвящен изучению причин неудач в программах экстракорпорального оплодотворения (ЭКО). Акцентируется внимание на оценке рецептивности эндометрия, дисбаланса клеточного и гуморального звеньев иммунной системы, аутоиммунных реакций и роли ангиогенных факторов роста в реализации программ ВРТ. Отмечается, что нормальное течение гестации зависит от состояния ангиогенеза, продукции ангиогенных факторов, контролирующих рост, развитие и регресс сосудов, а также стимулирующих пролиферацию клеток и тканей. Одними из основных активаторов ангиогенеза в органах репродуктивной системы женщин рассматриваются сосудисто-эндотелиальный фактор роста, плацентарный фактор роста и их растворимые рецепторы. Важная роль в процессе пролиферации и дифференциров-ки эмбриональных клеток, а также взаимодействии бластоцисты с эндометрием в момент имплантации отводится гранулоцитарно-макрофагальному колониестимулирующему фактору. Высказывается мнение о высокой прогностической значимости оценки системы гемостаза и ангиогенных факторов роста в развитии плацентарной недостаточности и преэклампсии с ранних сроков гестации.
