The purpose of the study was to compare the functional status of adolescents, depending on the measure of impact of the offense. The data received indicated lower functional status of convicted adolescents, especially those with para-suicide behavior. Adolescents who were used humane methods of punishment for committed offenses demonstrated changes in the cardiovascular and respiratory systems only in the form of significant increase of stress index, while other parameters remained normal. The purpose of the work is to estimate the status of autonomic nervous system in patients with chronic cerebral ischemia by means of the analysis of heart rate variability at various stages of disease. 120 patients with chronic cerebral ischemia aged from 45 to 65 took part in the research. The comparison group included 30 patients with an arterial hypertension and without chronic cerebral ischemia. Heart rate variability analisis included time-domain and frequency-domain methods of five-minute sequence of the electrocardiographic intervals registered in at rest and in or-thostatic probe. The results of research have shown, that autonomic disorders with prevalence of sympathetic nervous system accompany initial implications of chronic cerebral ischemia. The second stage of disease is characterized by depression of activity of both autonomic, and central regulation. The expressed depression of autonomic maintenance of regulation of heart rhythm of both from sympathetic, and from parasympathetic nervous system was observed at the third stage of chronic cerebral ischemia

For the purpose of diagnostic and treatment improvement it is necessary to investigate relationship between different pathogenetic mechanisms in essential hypertension. The aim of our study was the comparative analysis of the parameters of cardiac remodeling and neurohumoral mechanisms of heart rate variability (HRV) in patients with essential arterial hypertension. 100 patients with essential arterial hypertension were included in the study. All patients went through heart rate variability parameters and left ventricle characteristics evaluation. Time and frequency domains were decreased in patients with arterial hypertension. Excess sympathetic activation takes place, parasympathetic system is depressed and it shows pathogenetic basis of disadaptation. Left ventricle hypertrophy is associated with progressive decrease of HRV and its circadian fluctuations. Concentric left ventricle hypertrophy is characterized by the most significant decrease in time and frequency domains of HRV. 1/3 of hypertensive patients with normal left ventricle geometry have diastolic dysfunction and also some decrease in time and frequency domains. In conclusion, we found that cardiac remodeling in patients with essential hypertension seems to be related to the severity of impairment of cardiac autonomic control

We investigated the heart rate variability (HRV) parameters in patients with rheumatoid arthritis (RA) and assessed their relationship with disease characteristics. Twenty-three female patients with RA [age 48+/-7 (mean+/-SD) years] free of cardiovascular diseases and 23 age- and gender-matched healthy controls were evaluated. After careful clinical examination, the following parameters were obtained after 24-h Holter recordings: average of all normal-to normal (NN) intervals over the entire 24-h ECG recording (meanNN, ms); the standard deviation for the time between NN complexes (SDNN, ms); the standard deviation of the average NN intervals for each 5-min period (SDANN, ms) and the square root of the mean-squared differences of successive NN intervals (rMSSD, ms). We also assessed quantitative parameters of the Poincaré plot: the standard deviation of the points perpendicular to the line-of-identity (SD1, ms); the standard deviation along the line-of-identity (SD2, ms) and their ratio (SD12). HRV parameters excluding SD2 were significantly lower in patients with RA, than in control group (p<0.05). Significant correlations of SDNN and SDANN with swollen joints count, Ritchie articular index, disease activity score (DAS) and disease duration were found. SDNN also correlated with leucocyte count and smoking. SD1 significantly correlated only with disease duration. Relationships between SDNN and smoking, swollen joints count and DAS were confirmed using multivariate analysis. Our data indicate that in patients with RA reduced HRV is independently associated with high disease activity and smoking. HRV assessment may be useful as a part of cardiovascular risk stratification in RA patients.

An orthostatic test with frequency-controlled breathing (with a respiration period of 10 s) or spontaneous breathing was used to analyze frequency estimates of the heart rate variability (HRV) in the low-frequency (LF) and high-frequency (HF) ranges in young men and women. It was demonstrated that the spectral components of HRV bear no signs of sex differentiation, suggesting a uniform structural organization of the system of autonomic nervous control of the heart (SANCH) in humans. The LF component of the HRV spectrum is a marker of the functional state of the SANCH; it should be studied under conditions of controlled breathing at a frequency of 0.1 Hz. The HF and LF components of the HRV characterize the state of the SANCH at a given moment and do not reflect directly its adaptation reserve. The HF component of the HRV is interesting as a parameter that may be used for estimating the changes in the adaptation reserve of heart autonomic control. It is preferable to analyze this component in the absence of external disturbances in the LF range of the spectrum.

Frequency estimates of the heart rate variability (HRV) spectrum influenced by external periodic stimuli were studied in healthy subjects and patients with coronary heart disease (CHD). Sensory stimulation by periodic eye opening at a rate of 15, 10, 8, 6, or 5 times per minute, as well as spontaneous and controlled breathing at a rate of 15, 10, 8, 6, or 5 times per minute, was used. It was found that the spectral response to external periodic oscillations was determined by a frequency-dependent phenomenon, the maximal amplitude of heart rate variations being observed in the case of external stimuli at a frequency of 0.1 Hz. A resonance frequency in the 0.1-Hz range may be suggested to exist in the cardiovascular controls. Significant differences in the HRV frequency characteristics between CHD patients and healthy subjects were shown. CHD patients had a characteristic decline in HRV responses to external oscillations; the power of these responses did not depend on the frequency of external stimuli.

The specific features of the changes in the low-frequency (LF) and high-frequency (HF) bands of the heart rate variability (HRV) spectrum were studied in coronary heart disease (CHD) patients with different numbers of coronary arteries affected by hemodynamically significant stenosis in the course of a bicycle exercise test. Male patients (143) with CHD aged 49±8 years participated in the study. Subgroups with hemodynamically significant stenosis in one, two, or three coronary arteries; with no stenosis; and with a varying degree of total coronary bed lesion were distinguished. Tachograms were recorded in the course of bicycle exercise tests with loads of 25 and 50 W with spontaneous respiration for 3 min at each stage of a test. The frequency estimates of HRV in the LF and HF bands of the spectrum were obtained using the parametric method for constructing the spectrum based on the autoregressive model. It was shown that hemodynamically significant coronary stenoses significantly influenced the state of the mechanisms of autonomic regulation of the heart, which, in CHD patients with a three-vessel coronary lesion, is characterized by an extremely low adaptability.

The aim is the studying of the significance of the heart rate variability (HRV) characteristics during the interpretation of the veloergometric (VEM)tests results in patients with ischemic heart disease. 243 patients with the stable angina of the II-III functional class (49 ± 8 years) were involved in the studying. The evidence of coronary pathology was assessed with the help of selective coronary angiography. Results of VEM testing, Doppler echocardiography and spectral analysis of HRV were analyzed. Registration of R-R chains was performed during 3 min in the relaxed condition and in 90 sec after the beginning of 25 Watt stage of the VEM test. Frequency assessments of HRV were obtained with the help of the parametric method of spectrum construction. The results showed, that using of the HRV spectrum indices allows to raise the diagnostic reliability of VEM tests. Negative result of a VEM trial is characterized by the low level of reliability and cannot be used as an index of absence of the coronary channel damage.
