Aim: The definition of the relationship of clinical and laboratory features of acute myocardial infarction depending on the suffering of chronic tonsillitis. Materials and methods. The study included 54 patients with acute myocardial infarction suffering for 1-2 days. The collection of the anamnesis, assessment of clinical factors, inspection of the palatine tonsils, clinical and biochemical blood tests have been done. Markers of myocardial necrosis, an electrocardiogram with calculation of a dispersion of interval QT, echocardiogram have been taken into account. Results. 45 of 54 patients reported the symptoms of chronic tonsillitis in their lifetime. At 17 patients the previous diagnosis of chronic tonsillitis has been made, in 6 of them bilateral tonsillectomy was held. All of the patients were divided into 2 groups: 1 with proven chronic tonsillitis (17 patients) and 2 — the others (37 patients). At patients with chronic tonsillitis substantially more developed acute heart failure at sick this group glucose of the blood at receipt was higher than in 2 groups (p=0,004), given distinction was independent of presence of diabetes. According to the echocardiography 1 group of patients determined course-diastolic dimensions of the right ventricle increase in comparison with 2 groups (p=0,01). Conclusion. In patients with chronic tonsillitis more severe course of acute myocardial infarction has been determined, which became evident in the relatively high values of blood glucose on admission. The frequent development of congestive heart failure, and increase of the course-diastolic dimensions of the right ventricle have been also revealed.

Background: Synchronization between 0.1-Hz rhythms in cardiovascular system is deteriorated at acute myocardial infarction (AMI) leading to a disruption of natural functional couplings within the system of autonomic regulation. Objective: This study evaluates the prognostic value of autonomic regulation indices for the 5-year risk of fatal and nonfatal cardiovascular events in patients after AMI. Methods and Results: We studied 125 patients (53 [42%] female) after AMI aged between 30 and 83 years. The period of observation was 5 years with checkpoints at the first week after AMI and after each year after AMI. We compared the prognostic value of established clinical characteristics and degree S of synchronization between 0.1-Hz rhythms in heart rate and microcirculation for evaluation of the 5-year risk of mortality and recurrent myocardial infarction (MI) in patients after AMI. Acute heart failure Killip 2–4 at AMI and S < 20% at the first week after AMI were identified as the most important factors for evaluation of the risk of 5-year mortality in patients after AMI (χ2= 14.2, P = 0.003). Sensitivity and specificity of low S (<20%) at the first week after AMI were 76% and 43%, respectively. For evaluation of the 5-year risk of recurrent MI index S had no advantage over established clinical characteristics. Conclusion: The value of S below 20% in patients with AMI is a sensitive marker of high risk of mortality during the subsequent five years. It is characterized by better prognostic value than most of established clinical characteristics.

Aim: The aim was the studying of synchronization between 0.1 Hz oscillations in heart rate (HR) and plethysmographic peripheral microcirculation (PM) in acute myocardial infarction (AMI) patients and in healthy subjects. Material and Method: 12 healthy volunteers aged 26±5 years and 125 patients with AMI aged 65±9 years were involved in the study. Simultaneous registration of electrocardiogram and photoplethysmogram were performed during 10 min. In AMI patients the signals were recorded twice: the first record was done during 3-5 days after AMI, the second record was done during the third week after AMI. Phase differences between HR and PM oscillations were used to measure the degree of synchronization (S). Data are submitted as medians with inter-quartile ranges (25%, 75%). Results: S was 65.8% (50.5%; 79.5%) in healthy subjects whereas in AMI patients at the first week after AMI S was 16.3% (9.4%; 24.6%) (p<0.001). In records made at the third week after AMI index S was 18.4% (11.2%; 28.2%). Two groups of AMI patients were identified on the basis of individual S dynamics. In 100 AMI patients no dynamics of S was observed during the observation period and in 25 AMI patients the increase of S was observed. The group of AMI patients with increase of S had greater HR values during the first week after AMI. Conclusion: The index S of synchronization of 0.1 Hz oscillations in HR and PM appears to be a sensitive indicator of autonomic control dynamic disturbances in AMI patients.
