Современное понимание механизмов формирования хронической боли (ХБ) предполагает обязательное рассмотрение не только собственно сенсорных, но и эмоционально-аффективных и когнитивных ее аспектов. Когнитивные нарушения (КН) у пациентов с различными хроническими болевыми синдромами вносят существенный вклад в нарушение повседневного функционирования. В последние годы благодаря исследованиям функциональной нейровизуализации представления о механизмах формирования КН и эмоциональных расстройств у этой категории больных существенно расширились. Результаты экспериментальных исследований демонстрируют изменения как в структурах, относящихся к системам контроля боли, так и в областях мозга, ответственных за ключевые когнитивные функции. Эти данные расширяют представления о механизмах формирования полиморфной клинической симптоматики у пациентов с ХБ, а также обосновывают целесообразность использования психологических методов в комбинированной терапии ХБ.The current understanding of the mechanisms of chronic pain (CP) implies the mandatory consideration of not only its properly sensory proper, but also emotional, affective, and cognitive aspects. Cognitive impairment (CI) in patients with different chronic pain syndromes makes a substantial contribution to impairment in daily functioning. In recent years, the ideas about the mechanisms of CI and emotional disorders in this patient group have been considerably extended due to functional neuroimaging investigations. Experimental studies demonstrate that there are changes in both the structures related to pain control and the brain regions responsible for key cognitive functions. These data expand the views of mechanisms for polymorphic clinical symptoms in patients with CP and justify the feasibility of using psychological methods in its combined therapy.

The goal of the research was the comparative analysis of affective and cognitive disorders headache at women and men with dyscirculatory encephalopathy (DE), identify the connection of these disorders with the cerebral hemodynam-ics, neuroimaging of cinerea changes and BDNF in patients’ blood serum. 72 women (middle age 64,3±1,0) and 48 comparable in age and education men (middle age 65,0±1,2) with initiatory stages of dyscirculatory encephalopathy (DE) have been studied using comparative analysis of neuropsychological, sonographic and neuroimaging characteristics. The research revealed the predomination of cognitive impairment among men with (DE), especially attention, verbal coding and praxis disorders. Affective and insomnia disorders prevailed with women-patients. The results have shown possible links between low levels of brain-derived neurotrophic factor (BDNF) in blood plasma and the severity of depression and cognitive impairment at patients with (DE), especially at men. The received results may be important for individualization of therapeutic intervention.
