The given work summarizes external data and self-obtained results on development and diagnostic of kidney involution modifications. Article discusses definition of "senile kidney" as a clinical and pathomorphological term. Major statements on pathophysiological causes of age-associated renal disorders and their prognosis, specifics of chronic kidney disease in elderly and senile patients have been reviewed. Phenomenon of renal "multimorbidity" in eldely maximizes worsening risk of unmodifiable kidney function.

Introduction — Secondary hyperparathyroidism (SHPT) is common in patients with chronic kidney disease (CKD) affecting most of the patients in end stage renal disease. Materials and Methods — This prospective case control study was conducted on 50 adult patients of CKD, having individual parathyroid hormone (iPTH) levels >300 pg/ml. These patients were divided into two groups: Group I (n=25) received calcitriol in dosage of 0.5 µg/day. Group II (n=25) received cinacalcet in dosage of 30 mg/day and calcitriol 0.5 µg/day. The study was carried out for 6 month duration. Results — The mean basal value of iPTH decreased to 246.40±219.99 pg/ml at the end of the study from basal value of 482.80±229.86 in group II. There was a 43% decrease in serum iPTH values in group II as opposed to the 14% decrease in group I (P<0.05). Serum Ca×P decreased by 9% in group I and by 13% in group II and there was statistically significant difference at the end of 3rd month as well as at the end of study in cinacalcet group. Conclusion — Cinacalcet is unique in its ability to simultaneously lower PTH, calcium, phosphorus and Ca×P in patients with SHPT and thus could become an important component in treatment of CKD patients.

Aim: comparative investigation of cardos (antibodies to angiotensin II receptor subtype 1 (AT.,) C-terminal fragment), diovan (Valsartan) or both drug combination effects (changing of clinical picture, physical exertion tolerance and quality of life) for treatment chronic heart failure (CHF) patients. Methods. 12-month open-label randomized research was performed. CHF patients (NYHA Class l-ll, n=30) with concomitant chronic kidney disease (stage V, conventional hemodialysis) were randomized (10 patients in each group) for 6-month treatment by cardos (group I, average dose 1,8g/day), diovan (group II, average dose 80mg/day)orboth drug combination (group III, cardos 1,8g/day and diovan 80mg/day). CHD basic treatment was prescribed for all patients. In a 6-month drug crossover between groups I and I was performed, group III was divided into 2 subgroups (subgroup IIIA— cardos, subgroup NIB — diovan) followed by next 6-month treatment. Results. Long-term treatment by cardos has improved functional class (NYHA) of CHF patients with concomitant chronic kidney disease (stage V, conventional hemodialysis). cardos, diovan and both drug combination have demonstrated improvement of physical exertion tolerance, quality of life and patient clinical status during 6-min walking test. Conclusion. Cardos and diovan have shown the same efficacy. Cardos can be used as real alternative in case of ARA administration necessity
