Research goal: optimization of peripheral regional anaesthesia under ultrasound. Material and Methods. Study has been performed by SonoSite Edge (SonoSite, Bothell, США) on a lineal transducer machine for fixed terms 38-mm (13-6 MHz) of overall gain and depth. The study included 2 sonographic needles and 2 non-sonographic needles. Ultrasound images were obtained and analyzed by an experienced anaesthesiologist. Results. In this study improved ultrasound imaging needle for peripheral regional anaesthesia using technology SonoMBe compared with a conventional scanning mode has been demonstrated. Conclusions. SonoMBe technology increases the echogenicity of needles for peripheral regional anaesthesia that allows the use of this technology gain in the performance of nerve block and plexus block anesthesia

The studies of free radical blood activity in surgical patients of urological and gynecological profile, operated under regional anesthesia (RA) or total intravenous anesthesia (TIVA) have demonstrated that biochemical blood parameters, characteristics of stress reaction during anesthesia and early postoperative period, underwent less marked changes and normalized quicker during regional anesthesia. Early postoperative period in regional anesthesia coursed better than in TIVA.

The analysis of the results of local anaesthesia (LA) in 89 patients aged 18 to 68 years with trauma and diseases of extremities has been carried out. The efficiency of perineural injected multicomponent solution of clonidine added to conventional mixture oflidocaine and phentanyl has been assessed.
The multicomponent method has significantly prolonged the duration of local anaesthesia of 1% lidocaine solution providing reliable anaesthesia of plexus and peripheral nerves alongside the sufficient regional myoplegia and prolonged postoperative anesthetization. When there is no need of deep myoplegia Hallows to lower the concentration of lidocaine solution twofold, maintaining its efficiency, and to realize local anaesthesia of patients with high risk.
